Overview
HealthOne Rouse Hill, a collaborative between Hawkesbury-Hills Division of General Practice and Sydney West Area Health Service, is a NSW Health funded initiative that integrates primary health care services to better meet the health needs of people in NSW. This means that General Practitioners, community health services, other health care providers and community agencies work together to provide comprehensive, co-ordinated and collaborative care for patients to improve their health outcomes. The service is primarily for patients with complex health issues.
Service delivery is co-ordinated through the HealthOne Rouse Hill Hub, where a special interest GP with expertise in chronic disease management, along with a community nurse, conducts a 2 day/week clinic to provide assessment, consultancy, case management and regular communication with patient’s local GP. All patients will be required to provide informed, written consent for information exchange and enrolment in HealthOne.

Monday and Thursday 9am – 3pm.
Who Can Be Referred
Patients who reside within the designated postcodes. Click here for list.
Patients with:
- Diagnosis of chronic and complex illness
- History of 28 day re-admission
- Risk factors for older age
- Aged 75 plus years
- Risk of falls
- Cognitive impairment
- Reduced nutritional status
- 3 or more ED/Hospital presentations in past 12 months
- Acute exacerbation
- Lack of social support
- Major life event
- Severe end stage disease
